114學年度學位服登記借用 Cap and Gown Rental

114學年度申請學位服借用者，請填寫以下表單，填完之後務必按下「確認送出」!  如有問題，請先上網瀏覽借用注意事項，或請致電分機2371黃小姐，謝謝。

Please fill in the following form for renting the cap and gown for Fall Semester 2025 . After filling in the form, be sure to click  " Confirm and Submit" !

If there's any question, please dial extension 2371 for Ms. Huang. 


學碩士服請填寫XL尺寸所需數量 （尺寸：XL: 175公分以上）

Please fill in the number of XL sizes required for Master's and Bachelor's gowns.    （Sizes：XL: 175cm or above）Rented out

學碩士服請填寫L尺寸所需數量 （尺寸L: 168-174公分）

Please fill in the number of L sizes required for Master's and Bachelor's gowns.    （SizesL: 168~174cm ）

博士服請填寫L尺寸所需數量  (尺寸L:160以上)

Please fill in the number of L sizes required for Doctoral Gown.    （Sizes：L: 160cm or above ）

學碩士服請填寫M尺寸所需數量 （尺寸M: 162-167公分）

Please fill in the number of L sizes required for Master's and Bachelor's gowns.    （SizesM: 162~167cm ）

博士服請填寫M尺寸所需數量   (尺寸M:160公分以下)

Please fill in the number of M sizes required for Doctoral Gown.    （SizesM: under 160cm）

學碩士服請填寫S尺寸所需數量 （尺寸S: 161公分以下）

Please fill in the number of S sizes required for Master's and Bachelor's gowns.    （SizesS: under 161cm.）

(自2025/11/3起開始預約，格式為西元年/月/日(YYYY/MM/DD 並自備紙箱或大袋子)

Fill in Year/ Month/ Day. Box or bag shall be prepared by the renter.


博士服450元/件、碩士服300元/件、學士服250元/件。

Rental fee for each piece：

Doctor’s: NT$ 450.

Master’s: NT$ 300.

Bachelor’s: NT$ 250.


小提醒:需繳納完畢，才能拿取學位服。

Notice: the receipt of payment is required when picking up the gowns.

 

請附加電子檔並於領取衣服時繳交申請單紙本 (博士服申請單、學碩士服申請單)，謝謝。

Please submit the application form (Doctoral、Master's & Bachelor's) when picking up the academic gown. Please also submit a printed copy.
